What's happening
Your underarms contain thousands of eccrine sweat glands. These glands are controlled by nerves — when the nervous system signals them to produce sweat, they do. In hyperhidrosis, those signals are overactive. The glands get told to sweat too much, too often, even when the body is cool and at rest.
It's not caused by poor hygiene, being unfit, or eating the wrong things. It's a nerve-signal problem — the glands are being told to produce more sweat than the body needs. Stress, heat and certain triggers can make it worse, but the root cause is overactive nerve signalling to the glands.
Why antiperspirants and creams can't fix this
Strong antiperspirants (the ones with aluminium chloride) work by physically blocking the sweat ducts at the surface. But the glands are still being told to overproduce — so the sweat just builds up behind the blockage, and many people find the effect weak or short-lived. Some develop irritation from the strong chemicals needed.
Creams and topical treatments sit on the skin and can't reach the nerves that drive the overproduction. They can mask the problem temporarily, but they can't quieten the signal that's causing it. To reduce the sweating at its source, you need to interrupt the nerve telling the gland to overproduce.
How we treat it
The same prescription-only botulinum toxin used for wrinkles is injected in a grid pattern across the underarm skin. The toxin blocks the nerve endings that signal the sweat glands. Without the signal, the glands stop overproducing. You still sweat normally elsewhere — the treatment only affects the area where it's placed.
Why this treatment, specifically
Nothing else interrupts the nerve-to-gland communication. Antiperspirants block the ducts, not the signal. Creams can't reach the nerves. Oral medications act systemically (whole body) and carry side effects that aren't worth it for most people. Surgery (sympathectomy) cuts the nerves permanently — invasive and irreversible.

Anti-wrinkle injections for hyperhidrosis are one of the highest-satisfaction treatments we offer — most people wish they'd done it years ago. But it's not a cure. The treatment wears off, and you'll need top-ups to maintain the effect. Some people find the first treatment lasts closer to four months, with later treatments lasting longer as the glands become less active over repeated cycles.
If your excessive sweating is part of a wider condition (generalised hyperhidrosis affecting the whole body, or triggered by an underlying issue like thyroid), we may suggest speaking to your GP alongside the treatment — underarm injections won't address sweating elsewhere. We'll be honest about that at consultation.
